I understand that by submitting this form I am making a commitment to 100 Women Who Care Salt Lake City 

to make quarterly donations of $100 directly to local charities ($400 annually).

I understand that I am still responsible for making my donation:

- even if I did not vote for the charity chosen by majority vote

- even if I am unable to attend a quarterly meeting.

I can discontinue my membership at any time by emailing 100womenslc@gmail.com.
